Last updated: January 2025  ·  Version 1.0

These Terms of Service (“Terms”) govern your use of the Sams Care website and private prescribing services at samscare.uk. By submitting a request or using our services, you agree to be bound by these Terms. Please read them carefully before proceeding.

1. About Our Service

Sams Care is a private clinical prescribing service operated by Sepideh Sadig Baheran, NMC-registered Independent Nurse Prescriber, based in the United Kingdom.

Our services include: private prescription issuance, aesthetic prescribing for qualified practitioners, and V300 prescribing supervisor and assessor support.

Important: Sams Care is not a GP surgery, NHS service, or emergency medical provider. We do not replace your primary care physician. For emergencies, call 999. For urgent non-emergency medical advice, call NHS 111.

2. Eligibility

To use our services you must:

  • Be aged 18 or over. We do not provide prescribing services to individuals under 18 years of age.
  • Be a resident of or located within the United Kingdom at the time of your request.
  • Provide accurate, truthful, and complete information in your clinical intake form.
  • Not be seeking prescriptions for controlled drugs (Schedule 2 or 3), oncology medications, or medical cannabis.
  • For aesthetic prescribing: be a qualified healthcare professional with relevant training (NMC, GMC, GDC, or equivalent registration).

3. Clinical Scope and Limitations

Sams Care cannot and will not prescribe the following categories of medication under any circumstances:

  • Controlled Drugs (Schedule 2 and Schedule 3 under the Misuse of Drugs Regulations 2001)

  • Medications for patients under 18 years of age
  • Complex psychiatric medications (e.g. clozapine, lithium)
  • Medications during pregnancy or for gestational conditions
  • High-risk immunosuppressants outside of specialist care

We reserve the right to decline any request at our clinical discretion, without obligation to provide a reason, where we determine it would not be safe or appropriate to prescribe.

4. The Prescribing Process

By submitting a request you acknowledge and agree that:

  • All information you provide must be accurate and complete. Providing false or misleading clinical information is a serious matter and may constitute fraud.
  • A prescription will only be issued following a clinical assessment by our prescriber. Submission of a request does not guarantee issuance of a prescription.
  • We may request additional information, documentation, or evidence before proceeding.
  • Prescriptions issued are valid for 6 months from the date of issue unless otherwise stated.
  • You are responsible for ensuring the medication is dispensed and used safely and in accordance with the directions provided.

5. Fees and Payment

Our current fees are displayed on the website at the time of service selection. All fees are stated in pounds sterling (GBP) and are inclusive of any applicable taxes.

Fees are charged per prescription request. Payment is processed securely at the time of consultation booking.

Refund policy: If your request is declined on clinical grounds before a prescription is issued, a full refund will be provided. Refunds are not available once a prescription has been issued and emailed to you.

6. Follow-up and Aftercare

As part of our clinical duty of care, we will contact you by email at 7 days and 30 days following the issuance of a prescription to monitor your safety and wellbeing.

You are encouraged to respond to follow-up communications honestly. If you experience any adverse effects, please seek immediate medical attention and inform us.

7. Your Responsibilities

You agree to:

  • Provide complete and accurate medical history, including all current medications, allergies, and conditions.
  • Inform us of any changes to your medical history that may be relevant to your prescription.
  • Use prescribed medications only as directed and not share them with others.
  • Store medications safely and in accordance with the instructions provided.
  • Notify us immediately if you experience any adverse reactions or side effects.
  • Not misuse the patient token system to obtain prescriptions for third parties.

8. Limitation of Liability

Sams Care provides services in good faith based on the information you supply. To the maximum extent permitted by UK law:

  • We are not liable for adverse outcomes resulting from incomplete, inaccurate, or misleading information you provide.
  • We are not liable for how medications are stored, used, or administered after delivery of the prescription.
  • Our total liability to you in connection with any single prescription request shall not exceed the fee paid for that request.
  • Nothing in these Terms limits our liability for death or personal injury caused by our negligence, fraud, or any other liability that cannot be excluded under English law.
